ABSTRACT

This study evaluates the Patient Navigation Program (Programa de Navegação do Paciente - PNP), which was introduced to a community in the municipality of Rio de Janeiro. The objectives were: to establish the viability of the PNP in this context; identify barriers to mammogram screening; and ensure mammogram coverage for 70% of women recruited between 50 and 69 years old. From March to September 2018, 678 women with an average age of 58 years old were recruited from the Andaraí community. Follow-up was performed through the patient browser (PB), by telephone, email and text messages. Twelve percent of women refused to participate in the PNP for cultural reasons. The main barriers reported by women were: systematic problems with health care programming (100%), financial problems (64%), concerns about communicating with medical staff (58%), fear (44%), and social support (14%). The PNP obtained 100% satisfaction, and the mammogram coverage rate goal was exceeded, reaching 88%. The PN promoted an increase in the rate for mammogram coverage, aided in the transmission of quality information, reduced individuals' fear of mammography, and facilitated access to breast health care.


Este estudo avalia a introdução do Programa de Navegação do Paciente (PNP) em uma comunidade do município do Rio de Janeiro. Os objetivos são: estabelecer a viabilidade do PNP nesse contexto; identificar as barreiras ao rastreamento mamográfico; e assegurar cobertura mamográfica de 70% das mulheres recrutadas entre 50 e 69 anos. De março a setembro de 2018, foram recrutadas 678 mulheres com idade média de 58 anos da comunidade do Andaraí. O acompanhamento foi realizado pelo navegador de pacientes (NP) por telefone, e-mail e mensagens de texto. Doze por cento das mulheres recusaram-se a participar do PNP por razões culturais. As principais barreiras relatadas pelas mulheres foram: problemas do sistema com programação de cuidados de saúde (100%), problemas financeiros (64%), preocupações relacionadas à comunicação com a equipe médica (58%), medo (44%) e apoio social (14%). Foram obtidos 100% de satisfação com o PNP, e a meta de taxa de cobertura mamográfica foi superada, atingindo o percentual de 88%. O NP promoveu aumento na taxa de cobertura mamográfica, auxiliou na transmissão de informações de qualidade, reduziu o medo da mamografia e facilitou o acesso aos cuidados de saúde da mama.

Introducción: A hemisferectomía es un procedimiento valioso en el tratamiento de trastornos convulsivos causados por desordenes hemisféricos unilaterales. El hemisferectomía anatómica se ha utilizado para este fin desde 1938, sin embargo, se abandonó este procedimiento después de informes de complicaciones postoperatorias causadas por hemosiderosis superficial, ependimitis e hidrocefalia obstructiva. Así que, se ha mostrado en la literatura modificaciones en las indicaciones y técnicas de hemisferectomía anatómica cuya finalidad es la de reducir la incidencia de esta complicación sin dejar de lograr control de las convulsiones. Sobre la base de la literatura, la hemisferectomía mejora la calidad de vida de los pacientes que tiene la indicación para realizar este procedimiento, ya que permite reducir la frecuencia de las convulsiones, si tónica o átona, tónico-clónicas Objetivo: El objetivo de esta revisión de la literatura es discutir los detalles técnicos, modalidades, riesgos, complicaciones, resultados y de pronóstico de hemisferectomía basado en la revisión crítica de la literatura. Casuística y Métodos: Se realizó la consulta bibliográfica, utilizando la base de datos MEDLINE, LILACS, SciELO, que utiliza el lenguaje como criterios de selección, la elección de los artículos recientes preferiblemente en portugués, español o inglés. Conclusión: Según las referencias, hemisferectomía es un procedimiento con buen resultado para las personas con convulsiones derivadas cuando está indicado para casos seleccionados y la tasa de éxito no es proporcional a la extensión de la resección del tejido neuronal. A mayor resección puede o no reducir la frecuencia de las crisis, sin embargo, la incidencia de la morbilidad puede ser mayor.


Background: The hemispherectomy is a valuable procedure in the management of seizure disorders caused by unilateral hemispheric disease. The anatomical hemispherectomy has been used for this purpose since 1938, however, it was abandoned after reports of postoperative complications caused by superficial hemosiderosis, ependymitis and obstructive hydrocephalus. So that, it has been showed modifications in the techniques of hemispherectomy whose the purpose is reduce the incidence of this complications while still achieving seizure control. Based on literature, the hemispherectomy improves the quality of life of patients that has the indication to perform this procedure because it allows reducing the frequency of seizures, whether tonic or atonic, tonic-clonic. Aim: The aim of this literature review is discuss the indications, technical details, modalities, risks, complications, results as well de prognosis of callosotomy based on critical literature review and the authors experience. Casuistry and Methods: It was performed bibliographical consultation, using the databases MEDLINE, LILACS, SciELO, utilizing language as selection criteria, choosing preferably recent articles in Portuguese, Spanish or English. Conclusion: According to references, the functional hemispherectomy has a good outocome for those with seizures arisin when indicated to selected cases and the success rate is not proportional to the extent of neuronal tissue resection. So that, a greater resection cannot necessarily reduce the seizure frequency, however the morbidity may also be larger.

ABSTRACT

El número de casos de niños que sufren violencia doméstica está aumentando actualmente. La finalidad de este estudio fue analizar el conocimiento de los Líderes de la Comunidad sobre la violencia doméstica, cuán preparados están para identificarla; reconocer el órgano competente para la notificación; y si, notificada la agresión, son capaces de actuar en defensa del niño. Fue realizada una entrevista estructurada con los Líderes de la Comunidad Pastoral del Niño en la región oeste de la ciudad de Londrina-PR. Los resultados demostraron que: 96 por ciento no son capaces de identificar la violencia domestica la violencia para ellos corresponde solamente a golpear o gritar al niño, y 30 por ciento saben que el órgano notificador es el Consejo Tutelar, pero no lo hacen por miedo de alguna represalia. La conclusión es que existe la necesidad de se hacer grupos para esclarecer el tema, capacitación con los Líderes y reuniones con las familias.


The number of children's cases that you suffer domestic violence is increasing now. The purpose of this study was to analyze the degree of knowledge of the Community Leaders on violence tames, the all is prepared to identify her; the knowledge on which they are the competent organs for the notification; and if, notifying the aggression, they are capable of being acting on behalf of the attacked child. They took place an interview structured with Community Leaders of the Child's Pastoral in the area west of the city of Londrina-PR. The results demonstrate: 56% of the interviewees are capable to identify the domestic violence, where the violence for them only corresponds to "beat in the child", and still 30% of them know that the organ is Guardian Council don't do him for fear of some type. It is ended that the need exists of being if making groups for explanations on the approached theme, trainings with the Leaders and meetings with the families.
